Abstract

Title: Roles of Clinical Pharmacists in the Management of Infections in HSCT Patients
Body: Infections have a significant impact on morbidity and mortality in the peri- and post-HSCT setting. Patients proceeding to HSCT have impaired immunity due to the primary disease, initial myelosuppressive therapies, and previous infections. The presence of myeloablation, reconstitution of a new immune system, use of immunosuppressive drugs and GvHD are independent risk factors for infection complications. Pharmacist is part of multidisciplinary care team in management of infections in HSCT patients and plays role in various aspects include drug allergy history taking, management and monitoring of antimicrobials administration, therapeutic drug monitoring, management of antimicrobials with special approval, antimicrobial stewardship program and education. These activities improves the overall patient's therapeutic outcome.
